Решение
Документация Qt для setItemDelegateForRow довольно ясна по этому поводу:
Any existing row delegate for row will be removed, but not deleted.
QAbstractItemView does not take ownership of delegate.
Итак, вы уже диагностировали свою проблему, но не выполнили ее.
Сегфоулты происходят потому, что некоторые из делегатов собираются сборщиком мусора до инициализации табличного представления.
Попробуйте создать подкласс табличного представления, в котором хранится внутренний список его делегатов:
